The Milwaukee MIL49-56-9645 is a bi-metal hole saw, 4 inch diameter, 1-5/8 inch cutting depth, 3.5 TPI, 3/8 inch hex shank, with a 1/4 inch diameter x 3-1/2 inch long pilot drill.
The 1-5/8 inch cutting depth is the specification most people miss and then discover, and it is worth being explicit about. A hole saw cuts a ring — it does not remove the middle. That means the material being cut must be thinner than the cup is deep, or the plug bottoms out inside the saw and the cut stops. Four inch holes through a 2x6 are not going to happen with a 1-5/8 inch cup without cutting from both sides.
The pilot drill is the other essential component. A 4 inch toothed ring against a flat surface has nothing to locate it and will skate across the work. The pilot enters first and holds the centre while the teeth engage.
At 3.5 TPI this is a coarse pitch — for wood, plasterboard, and soft materials where large chips need to clear. Metal needs a fine-pitch saw.
The real hazard is kickback. A 4 inch hole saw grabbing generates enormous torque reaction, and it will twist a drill out of an unprepared grip. Use a side handle and expect it.
