A pin spanner turns a fastener that has no flats at all, and understanding what those fasteners look like explains the tool.

A great many round retaining nuts — bearing lock nuts, collars, hydraulic caps, spindle nuts, pump housings — are cylindrical with holes drilled in them. There is nothing to put a wrench on. The holes are the drive.

A pin spanner has a pin that drops into one of those holes and a handle to turn it. Adjustable means the pin's distance from the pivot can be set, so one wrench serves a range of hole circles rather than needing a dedicated tool per fastener.

The specification that decides whether it works is the pin diameter against the hole. A pin that is too small rocks in the hole and rounds it out — and a rounded drive hole in a bearing retainer is a serious problem, because there are usually only two holes and no other way to turn it. Too large and it will not enter.
