The three inch offset is the number to do something with, and this is a good place to show what.
The correction formula for an interchangeable head is straightforward: the wrench setting equals the desired torque multiplied by the wrench's lever length, divided by that length plus the head's offset.
Put numbers on it. Suppose a 15 inch wrench and a desired 80 ft-lb. With this head's 3 inch offset, the effective length becomes 18 inches. The setting must be 80 × 15 ÷ 18, which is about 67 ft-lb. Set the wrench to 67 and the fastener receives 80.
Set the wrench to 80 instead — which is what happens when nobody does the arithmetic — and the fastener receives about 96. That is twenty percent over, on a torque-critical joint, with the wrench clicking confidently.
That is the entire reason the offset is printed on the listing.
The open end head is what reaches a fitting mid-run, where a box or a socket cannot slide on.
The 0.560 inch Y shank is the middle torque class here — 99 ft-lb, against about 60 for a J shank and 240 for an X.
